Youth Corrections - Savio Transition Program (STP) Developed in close cooperation with the DYC Regional offices, the Savio Transition Program is a systemic approach to serving youth who are transitioning from long-term placements to the community. Youth are referred to the STP program 120 to 60 days prior to parole. The 120 to 60 day time frame allows for relationship building and thoughtful planning before the youth is released. There is time to work with all the possible services on which the transition plan may draw, thus preventing gaps in service. Some of these services will begin many weeks or months prior to release, while others don't come into play until after the client is released. When the STP Supervisor receives the referral from the Client Manager, an STP Specialist is assigned to the youth. The Specialist maintains knowledge of and relationships with area community resources including landlords, employers, mental health services, mentors, community recreation and neighborhood retail and banking resources. Specialists are assigned to designated regions so they become knowledgeable about the unique needs and strengths of that community. A critical relationship maintained by the STP Specialists is with the regional mental health providers. Prior to parole the Specialists are able to coordinate community mental health services for youth, avoiding lapses in services and streamlining access to medication management. Within two weeks of referral, the STP Supervisor arranges an initial staffing. The initial staffing involves the youth, family, Client Manager, STP Supervisor and/or Specialist, and DYC facility staff, and includes an assessment utilizing the Colorado Juvenile RISK Assessment. An individualized plan is developed by the transition team to identify the combination of services needed to support the youth. The youth may be referred to a variety of community services both formal and informal. Many of these services are available through Savio's DYC Provider Network. Youth who need more individualized community supports are assigned an STP Worker. Parolees who have a greater variety and severity of risk factors identified by the assessment are more likely to need a Worker. The Worker's role is to connect the youth with long-term sustainable resources that will remain engaged with the youth even after formal services are complete. If the youth's family is involved, STP services will include teaching parents to build skills and access resources on their own for continued support. |
